To register or make an appointment for physical or occupational therapy rehabilitation, depending on your insurance, you may need a referral from your family doctor, nurse practitioner, dentist, podiatrist, or chiropractor. When calling, please inform our staff of the type of health insurance you have so we can verify and assure your coverage for your upcoming visit. You may print and fill out our new patient forms from the links below, or request that we e-mail our new patient forms so that you may fill them out prior to your first visit and bring them in with you.
